Dina Smith, CPhT., Senior Analyst
Dina is a nationally certified pharmacy technician responsible for system-based medication error investigation, interpretation and evaluation. Her expertise includes the review and qualification of Retail Pharmacy Unit Customer Relation’s Complaints specific to Quality Assurance and Safety issues. Dina is instrumental in the research and documentation of root cause for medication error events.
